    Family Medicine APP

    StaffMed Health Partners has partnered with a physician-owned and operated practice located in the greater Oklahoma City area to assist them with the recruitment of a Family Medicine APP to conduct in-home visits for transitional care and primary care as part of their house call program in Edmond, OK. Edmond is a northern suburb of Oklahoma City and was recently ranked the third safest and most affordable city in the nation. This opportunity will be in a brand new clinic working alongside all necessary support staff.

    Job Description

    • Provide primary care services to patients in their homes
    • Coordinate with the healthcare team
    • Conduct assessments and develop care plans
    • Ensure quality patient care

    Requirements

    • Board Certified NP/PA with active license with prescriptive authority in the state of OK
    • Experienced primary care experience preferred
    • Ability to work autonomously and collaboratively in a team environment
    • Excellent clinical assessment and diagnostic skills
    • Strong communication and interpersonal skills
    • EMR/EHR proficiency
    • Compassionate and patient-centered approach
    • Round at nursing home as needed
