    Hospice RN

    Stella Maris is proud to provide central Maryland with compassionate hospice home care for adults. We specialize in hospice services that enable the patient to adapt and manage his or her medical situation in the comfort of their home. Stella Maris offers home hospice services to residents of Baltimore City, Baltimore County, and Harford County.

    We are growing our team and looking for a Full-Time Hospice RN. Hours are from 7:00 am to 7:30 pm. If you are interested in learning more, apply today!

    Job Description

    • Provide hospice nursing and palliative care to terminally ill patients.
    • Ensure the highest level of care is provided to patients as they transition to end-of-life care.
    • Perform daily assessments of patients' physical and emotional needs with appropriate intervention and management of symptoms and coordination of other disciplines.
    • Enter all required documentation into EMR, including daily clinical notes, effectiveness of interventions/treatment plan, updating care plan, updating medications when necessary, and assessment if indicated.
    • Administer medications in accordance with policies and procedures and in compliance with federal and state regulations, following the 7 Rights of Medication administration.
    • Demonstrate ability, accuracy, and knowledge for transcribing physician's orders to MAR and complete monthly turnover of MARs.
    • Complete accurate narcotic counts at shift change and properly dispose of controlled medications upon patient's death or discharge following DEA regulations.
    • Complete 24-hour chart check for accuracy of orders and transcription.
    • Pass an annual medication test.
    • Complete an accurate and thorough shift report.
    • Notify MD/CRNP of changes in the condition of patients.
    • Complete comprehensive admission assessments on all new admissions.
    • Complete documentation for weekly Team Meetings and participate by providing accurate and current information on the patient.
    • Demonstrate competency in the use of IV - Central & Peripheral lines, subcutaneous therapies, IM injections, PICCs, PORTS, basic TPN/PPN, and PCA pumps.
    • Demonstrate competency in ostomy, drains, foley catheters, macy catheters, and wound care.
    • Demonstrate competency in trach care, CPAP, BiPAP, and high-flow oxygen.
    • Assess, implement, and evaluate respiratory issues and treatments competently.
    • Demonstrate competency with G-Tubes, NG Tubes, and tube feedings.
    • Follow infection control policy and procedure.
    • Demonstrate good body mechanics.
    • Delegate patient care tasks appropriately.
    • Supervise and provide direction to aides on the care delivered and provide feedback for the aides' annual performance appraisals.
    • Perform other duties within the scope of job functions as assigned.

    Requirements

    • RN licensure in the State of Maryland
    • Current Cardio Pulmonary Resuscitation (CPR) certification
    • Prior experience in hospice required
    • Certified Hospice and Palliative Nurse (CHPN) a plus
