    Clinical Pharmacist

    Become an essential part of patient care as a Clinical Pharmacist! Are you a skilled clinical pharmacist looking to make a direct impact on patient care in a hospital setting? We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our multidisciplinary team and play a crucial role in ensuring safe, effective, and individualized medication therapy for our patients.

    As a Clinical Pharmacist, you will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. You'll provide critical expertise in drug therapy management, medication reconciliation, and patient education, ensuring that our patients receive the highest standard of care. The Clinical Pharmacist collaborates with other healthcare providers to ensure appropriateness of all elements of pharmaceutical care and independently operates within policies and procedures to manage the department's clinical and consultative services. The Clinical Pharmacist supervises the work of supportive team members to ensure accuracy and efficiency of distributive services. Additional responsibilities include, but are not limited to serving as a drug information resource for TriHealth providers, direct patient interaction, precepting of pharmacy residents and/or student pharmacists, and contribution to department projects or initiatives in collaboration with Pharmacy Leadership.

    Job Description

    • Collaborate with healthcare professionals to optimize medication therapy and improve patient outcomes.
    • Provide expertise in drug therapy management, medication reconciliation, and patient education.
    • Supervise supportive team members to ensure accuracy and efficiency of distributive services.
    • Serve as a drug information resource for providers.
    • Contribute to department projects or initiatives in collaboration with Pharmacy Leadership.

    Requirements

    • Bachelor's Degree in Pharmacy.
    • Active pharmacist license in the state of Ohio.
    • Working knowledge of all Ohio Board of Pharmacy regulations.
    • 1-2 years of experience as a pharmacist is required; inpatient or infusion pharmacy experience preferred.
    • Working hours/days are every Tuesday and Wednesday from 7:30 AM - 6 PM. Required to work every 3rd weekend and holiday.
