    Adult and Senior Care Physician

    Location: Lancaster County, Pennsylvania

    UPMC Central PA is seeking per diem adult and senior care physicians to join our growing post-acute care team and play an integral role in helping to provide high-quality, compassionate care to our geriatric population. UPMC Central PA Post-Acute team provides care in several skilled nursing facilities across Lancaster County.

    Job Description:

    • Provides direct patient care in multiple facilities across the aging continuum to include skilled nursing, rehab, personal care/assisted living, independent living, and clinic
    • Interacts with facility leadership, hospital discharge team, care management, and other specialties
    • Collaborates with and supervises Advanced Practice Providers
    • Utilizes EPIC EMR, online diagnostics including PACS imaging and laboratory results
    • Participates in on-call rotation, all phone coverage, shared equally with partners
    • Monday - Friday (No weekends)

    Requirements:

    • Board certified/Board eligible
    • Team-oriented with the ability to provide personalized patient care and demonstrate clinical outcomes
    • Commitment to providing high-quality and compassionate care to patients
    • Eligible for licensure in the state of Pennsylvania
