**Job Description**

- Manage all aspects of patient care from initial admission assessment to weekly visits, to discharge when goals are met.
- Coordinate patient care with their physician as well as other disciplines involved in care (PT, OT, ST, HHA).
- Upon admission, develop a plan of care tailored to the patient's individual needs, including establishing goals and discharge planning.
- Revise the plan of care as necessary, depending on patient status and any changes.
- Communicate with the physician regularly on patient condition and to request any orders necessary.
- Provide direct nursing care and education based on the care plan.
- Complete all assessments and nursing notes timely and thoroughly.
- Collaborate with other home health disciplines for the best patient outcomes, which includes attending case conference meetings regarding patient overall care.
- Supervise HHA and LPN visits when applicable.
- Communicate effectively with other team members, including administrative supervisors regarding scheduling, patient updates, or any other daily updates.
- Participate in on-call rotation when applicable.
