Said documentation shall be completed immediately but in no event later than the end of the applicable shift. Obtain verbal report from facility staff passing on patient care to you and obtain transfer or discharge summary. Provide verbal report to facility staff when passing patient care on to another individual and obtain appropriate signature. Must follow all Procare's policies and procedures as referenced in Procare's Employee Handbook.
