**Family Nurse Practitioner**

Mountain Community Health Partnership (MCHP) is seeking a full-time Family Nurse Practitioner to join our team based in Mitchell County in Western North Carolina. This is an outpatient-only position with no inpatient rounding. Our providers offer preventative care for children and adults, treatment of chronic and acute conditions including same-day walk-in visits. The medical provider will adhere to evidence-based guidelines as appropriate and participate in quality improvement activities. The FNP will have a large scope of practice, including direct provision of care. In addition to providing outpatient medical care, duties include, but are not limited to, complying with all MCHP goals for the practice. They must display knowledge of normal signs of human development and the ability to assess and provide age-appropriate and culturally sensitive care. The FNP must maintain the privacy of all patients, employees, and volunteer information, accessing such information only on a need-to-know basis for business purposes. Finally, utilizing a team-based approach, they are responsible for providing comprehensive, high-quality, and culturally sensitive rural primary care.

The FNP will maintain current knowledge of policies and procedures as they relate to safe work and practices. The position may require the FNP to see patients at more than one site.

Duties include, but are not limited to, serving as a direct provider of behavioral health services providing quality, compassionate, and culturally sensitive care for patients of MCHP; Completion of screenings and assessments, including diagnosis and mental status exams using appropriate clinical tools, clinical interviews and other collateral information; Supporting referrals, and formulating treatment strategies; Implement treatment plans with patients/students, working together to accomplish collaborative goals by frequent monitoring and face to face interactions; Provide consultations services to families, schools, legal and judicial systems, physicians, and other community agencies to assist these resources in understanding the need of the patients/students; Participation in scheduled team meetings. Work location will be hybrid (school setting and office setting) and will be based in Mitchell County, but willing to travel to other locations as needed. Must have a valid and current LCSW/LPC licensure; Two-year clinical experience, Electronic Health Record experience, Minimum of two years supervised employment preferably in primary care and Experience in individual and group therapy including recovery work preferred. The Pharmacist also functions in a consulting role and co-visit model with providers in a primary care setting. Consulting includes addressing medication access barriers, drug information questions, and managing high-risk patients. Other duties of the Clinical Pharmacist include but are not limited to chronic disease management via collaborative practice agreement and education for health conditions (i.e. diabetes, asthma, hypertension, heart failure, and cardiovascular disease), interpreting and evaluating appropriateness of patient medications, assist with management of 340B program, administer and monitor log acting injectables (i.e. Sublocade, Brixadi, antipsychotic, etc.), and other duties as assigned.
