**Registered Nurse - Full Time - Night Shift**

Raleigh Oaks Behavioral Health in Garner, North Carolina, is seeking a dedicated Registered Nurse responsible for providing nursing care to patients. This role involves supervising care treatment and services provided by LPNs, CNAs, and other staff on the assigned unit and shift. The Registered Nurse will develop, implement, and evaluate individual care plans for each patient, assess and re-assess patients, and communicate pertinent information with the interdisciplinary team.

**Job Description**

- Provide and coordinate care by assessing physical and behavioral health needs of patients, developing and implementing nursing care plans, maintaining medical records, and educating patients and their families about various health conditions while ensuring patient safety at all times.
- Assess patient's status on an ongoing basis and document pertinent information.
- Ensure nursing treatment plans are specific to assessed needs and complete the initial treatment plan upon admission.
- Accurately take physician's orders (verbal or via telephone) and document appropriately; transcribe and review physician's orders ensuring legibility.
- Administer medications per ordered note and document accurate counts of controlled substances; adjust medications under provider's supervision as needed and monitor lab results for medication compliance.
- Notify physician of significant changes in patient status such as difficulties with detox, medical emergencies, or changes in response to medications.
- Supervise self-administration of medications using two means of patient identification; document and report effects of scheduled and PRN medications.
- Maintain accountability for administration, recording, control, and security of medications, including narcotics.
- Complete admission and discharge processes efficiently and accurately as directed.
- Confer regularly with the treatment team regarding patient care; attend and participate in unit activities such as therapy groups, family meetings, staffing, and unit meetings.
- Provide education/explanation to patients, families, and co-workers regarding medication usage/maintenance, common side effects, patient rights, and other topics necessary for optimal patient care.
- Monitor patient acuity, census, staffing needs, and arrange for coverage as needed following established guidelines.
- Ensure thorough, accurate documentation is completed for all patients on each shift; complete daily patient assignment sheets and carry out assigned tasks.
- Give thorough reports to oncoming staff, supervisors, and physicians.
- Provide coverage in emergency services as needed.

**Requirements**

- Graduation from an approved/accredited school of nursing.
- One or more years of nursing experience within the clinical area of service provided by the facility preferred.
- Experience in behavioral health/psychiatric/addiction recovery environment preferred.
- Valid current RN license from the state or from a compact state, where applicable.
- Certified in Psychiatric Nursing preferred.
- CPR and de-escalation and restraint certification required (training available upon hire).
- First aid certification may be required based on state or facility requirements.
