- Patient population focuses on seniors (65+), Medicare-focused
- Hybrid schedule: 4 days per week onsite (8:00 AM - 5:00 PM) and 1 day per week remote
- Monday-Friday schedule with a part-time option of a minimum of 4 onsite days per week
- Light call requirements: 2 evenings per month and 2 weekends per year
- Patient volume of 15-18 patients per day

Comprehensive, team-based primary care with a strong emphasis on holistic and social support:

- Routine primary care and chronic disease management
- In-clinic and virtual care options
- Care coordination with specialists and hospitals
- Assistance navigating insurance and the healthcare system
- Focus on social determinants of health (housing, food security, loneliness)

**Care Team Structure**

Each patient is supported by:

- Primary Care Physician
- Gather Guide Partner (social support & community resources)
- Medical Technician Partner (in-home care and monitoring)

**Compensation & Benefits**

- Base salary: $250,000 - $290,000 (DOE)
- Bonus: 10%
