**Job Description**

- Direct all daily patient referral and intake operations including implementation of intake policies.
- Ensure compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
- Screen all referred patients to meet home health eligibility criteria or admission criteria.
- Direct the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Establish and maintain positive working relationships with current and potential referral sources.
- Ensure seamless transition of patients to home care by providing patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
- Ensure maximum third-party reimbursement through insurance verification and authorization processes.
- Plan & organize work assignments and set priorities. Identify opportunities for additional or improved services to meet customer needs.
- Maintain comprehensive working knowledge of ANX Home Healthcare contractual relationships and ensure that patients are admitted according to contract provisions.
- Maintain comprehensive working knowledge of community resources and assist referral sources in accessing community resources should services not be provided by ANX Home Healthcare. Redirect or refer patients to other resources when the agency is unable to meet care needs.
- Communicate and coordinate with the home health staff in providing the most accurate information to initiate the first home health visit.
- Behave in a courteous, professional manner with patients, staff, and health care professionals and maintain patient confidentiality.
- Participate in Departmental Quality Assurance Activities and Utilization Review activities as requested.
- Participate in in-service programs. Select and attend education programs to keep current in the area of practice.

**Requirements**

- Minimum experience of one (1) year as a home health LVN.
- Current professional license as an LVN in California.
- Ability to access electronic databases to obtain or enter pertinent patient information.
- Ability to work in a fast-paced environment with multiple interruptions.
- Knowledge of governmental regulations, Joint Commission standards, and private payer practices.
- Demonstrates good written and verbal communication skills. Demonstrates good negotiation and public relations skills.
- Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
